Grout letting go from tiles ?

So .... I'm doing some work at this office and got talking with the folks about why the grout is letting go from the tiled floors.

Any ideas? ;)

My suspicion is movement in the substrate, however seeing the tile spacer that was left behind it could also be either bad grout OR a bad grout installation.

Pick your poison.

Spacer in second photo and ghosts of spacers in first left behind and grouted over. Since spacers left behind oozed mastic or thinset left behind in grout joints and with spacers on top probally didn't cure completely before grouted.

Grouted right up to metal doorway and wall tile didn't use caulk or grout caulk for transitions and no thermal break between metal doorway to grout and tile. Wall tile grouted to wall and to floor no grout caulk line movement pressure and grout fails weakest joint line filled with spacers and thinset first edge in from wall.

Didn't wet tiles or use additive to grout and grout didn't cure right, dried out too fast.

Didn't use sanded grout for wide joints.

Floor not stable enough for tile.
