Patio removal/replacement

We have a small enclosed patio area in the back of our townhome between house and garage - a concrete slab covered with slate tile. Also a small grassy area and two small garden areas. 1. Due to aspen trees (now removed) being planted in our yard, the concrete slab has lifted and cracked in many different ares. Water leaking into the cracks makes it worse. 2. The tile is indoor slate tile and grout so that all of the tile is peeling off and coming off. 3. Because we live in connected townhomes with fences between the yards, we need to make sure the grading is exactly right so that neither our foundation or our neighbors' foundations are adversely affected. 4. We don't know what the best solution is for the patio materials - pressed concreted, bricks, new concrete pad. We would like something: a.. slip resistant since it contains the walkway between the garage and house and often has snow on it. b. durable and easy to maintain c. smoothish as we have a one year old and we would like him to be able to play d. to maintain a garden area and maybe a grass area
