What to do when there is no sub-floor under bath ceramic floor tiles?

The builder (1950) did not put sub-floor at all over studs under first floor bath ceramic floor tiles. To replace the tiles now, what happens when there is a differential in floor height when sub-floor is laid and new ceramic tiles are installed? Is it necessary to hire a plumber to adjust piping for the new sink? And how will the old toilet be able to be reinstalled over the existing pipe without an adjustment there for the change in floor height?

Also this change would mean that if I ever want to change the wall tiles later, wouldn't that present a problem?

What is the seriousness of the potential fact that there may not be any sub-floor under the bath tub either?
