I tried to use the clean-outs on the PRV, but that did not fix the issue. I then replaced the PRV with an identical match and it absolutely fixed the issue.

I bought it in store at Lowes for $70. They had another 3/4" PRV for ~$50, but it was not an identical match and would have been a little bit more involved. All and all, I already had the wrenches and tape, so $70 total and about 1 hour to fix wasn't too shabby.
