I won't argue your statement, but I'm 68 years old and have never seem a residential water heater set up so both elements can come on simultaneously.

I dont think I ever looked at how one was wired internally, manufactuer says [email protected] 240v thats what it gets, when my hw heater stops heating hot water I call a plumber ironically.

Consider myself now educated ... kinda

Usually, a residential grade box store type water heater is factory set for 240/30A. Next time you install a water heater check out the nameplate and wiring diagram for the possible factory configurations. Note: you cannot field change many hot water heaters because of the thermostats.
