Debris in toilets' reservoir tanks and constant trickling?

Hello All:

I am a single female college instructor who owns an old Portland style house in the historic Sellwood antique district of Portland, Oregon. The original house was built in 1903, but I have had extensive remodelling done by licensed contractors. However, both downstairs and upstairs new toilets trickle constantly and there seems to be a little debris that accumulates gradually in the upstairs tank reservoir; replacing plungers et al. does not fix the problem.

Can you help?

Is the tank over filling? That is is the water level all the way up to the top of the overflow? The debris is not uncommon, it could be sand in the water supply, or deposits coming loose from the insides of pipes anywhere in the water system.
